Job Summary


On a day to day basis, you will empower young people to take control of their health and well-being through your skills in social prescribing.

This approach connects young people to community groups and statutory services that provide emotional and practical support for patients, their families, and carers to take control of their health and well-being, improve their health outcomes, and maintain a healthy lifestyle.


You will provide hands on care and support to young people and their families and will be actively involved in the safeguarding of children in Workington working with our safeguarding lead GP.


You will work closely with the Workington GP Practice working alongside young people, supporting them to tackle loneliness, isolation, anxiety and depression by connecting them into a broad range of services and interventions to improve their health and well-being.


Duties & Responsibilities

  • To act as a main point of contact for young people accessing the service
  • Provide personalised support to children and young people to enable them to take control of their wellbeing, live independently and improve their health outcomes
  • Help young people identify the wider issues that impact on their health and wellbeing such as debt, poor housing, being unemployed, loneliness and caring responsibilities
  • Meet with young people on a one to one basis and give them time to tell their stories and focus on what matters to them, building trust and providing nonjudgemental support, respecting diversity and lifestyle choices.
  • Signpost young people and their families to all sources of help available
  • To meet with families and young people in the surgery, at home or at school to establish their needs and situation
  • Form links with the local schools and other community enterprises
  • Build relationships with key staff in the practice and PCN as well as external services to promote the social prescribing model
  • Liaise with Health Visitors, School Nurses other health care professionals
  • Liaise with appropriate statutory and voluntary sector agencies working in Education, Public and Mental Health, Primary and Secondary Care, Social Care and Youth offending teams and build up appropriate networks that promote timely access to support for young people
  • Work closely with the practice children's safeguarding lead GP, attend children's safeguarding practice meeting to ensure any issues and concerns are escalated and dealt with quickly and effectively and be expected to compile reports as needed
  • To respond to section 47 enquiries with support as needed
  • Manage and prioritise your own caseload, in accordance with the needs, priorities and support required by individuals on the caseload
  • To action referrals to children social care and early help and follow up referrals as needed
  • Where required refer patients back to other health professionals within the PCN/Practice
  • Refer young people back to other health professionals/agencies when their needs are beyond the scope of the link worker role e.g. when there is a mental health need requiring a qualified practitioner
  • Manage and prioritise your caseload reflecting your patients' needs
  • Ensure data is captured in the clinical system and appropriate templates are used.
  • To attend mandatory training and abide by policies and procedures
  • To carry out, from time to time, any other appropriate duties in line with capabilities as directed by the line manager

They may also have access to information relating to the practice as a business organisation. All such information from any source is to be regarded as strictly confidential.
  • Information relating to patients, carers, colleagues, other healthcare workers or the business of the practice may only be divulged to authorised persons in accordance with the practice policies and procedures relating to confidentiality and the protection of personal and sensitive data Provide support for Service Delivery Leads where required.
