Calling all Band 4 Associate Practitioners
We are keen to recruit an experienced Associate Practitioner to provide input to our CMHTOP care home pathway and dementia post diagnostic clinic.
You will be joining a team of occupational therapists, nurses, psychologists and Associate Practitioners/Associate Psychologists in providing specialist input to care homes in the local area.
The post holder will work as a valued team member by delivering health and/or social care interventions that focus on the care needs of people with dementia who are living in care homes.
They will work autonomously within the clearly defined boundaries of their roles, which will include acting as Lead Professional for a range of people using services.
