Description

Collaborative Working Relationships Actively work toward developing and maintaining effective working relationships both within LPC and with practices and other stakeholders Foster and maintain strong links with all services across locality Explore the potential for collaborative working and takes opportunities to initiate and sustain such relationships General Duties and Responsibilities To promote LPCs vision and values by upholding these in the day-to-day requirements of the role To enhance own performance through continuously developing own knowledge, skills and behaviours to meet the current and future requirements of the role and respond to the learning needs of LPC To undertake Mandatory Training within required timescales To regularly review own practices and make changes in accordance with current and/or best practice, make suggestions for improved practice and identify where other activities affect own practice To raise concerns around malpractice or wrongdoing at work, which may affect patients, staff or the organisation, at the earliest opportunity.

All Staff (at all levels) have the responsibility and duty of care to safeguard and promote the welfare of children and vulnerable adults by ensuring they attend training and are aware of, access and act upon the relevant policies, procedures and practice guidance appropriate to their role.

All employees are required to access Safeguarding Awareness training and undertake any additional service specific training and supervision as appropriate to their position.

Health and Safety/Risk Management The post-holder must comply at all times with the LPC Health and Safety policies, in particular by following agreed safe working procedures and reporting incidents using the organisations Incident Reporting System.


The post-holder will comply with the Data Protection Act and the Access to Health Records Act Equality and Diversity The post-holder must co-operate with all policies and procedures designed to ensure equality of employment.

Co-workers, patients and visitors must be treated equally irrespective of gender, ethnic origin, age, disability, sexual orientation, religion etc. Respect for Patient Confidentiality All members of staff are bound by the requirements of the Data Protection Act 1998.


Information relating to patients, carers, colleagues, other healthcare workers or the business of LPC may only be divulged to authorised persons in accordance with LPC policies and procedures, IG regulations and Data Protection Act relating to confidentiality and the protection of personal and sensitive data.

Development Review Key performance objectives, development needs and completion of a Personal Development Plan will be discussed and agreed at Annual Appraisal Reviews.

Job Description Agreement This job description is intended to provide an outline of the key tasks and responsibilities only. There may be other duties required of the post-holder commensurate with the position.


This description will be open to regular review and may be amended to take into account developments within the primary care.

All members of staff should be prepared to take on additional duties or relinquish existing duties in order to maintain the efficient running of primary care.
