As an integral member of our clinical management team, you will play a crucial role across our renowned MWL Frailty service.


With your advanced clinical practice skills, you will have the autonomy to make critical decisions and provide exceptional care to our patients.

From the moment you take their history, perform assessments, make diagnoses, deliver treatments, and evaluate their progress, you will be responsible for the comprehensive management of older individuals and those living with frailty.


As an ACP, you will not only excel in providing outstanding patient care but also take the lead in educational sessions, clinical supervision, research, audits, and driving quality and service improvements.

With opportunities to rotate across our Frailty offer, you will constantly expand your knowledge and expertise.

  • Provide advanced levels of clinical practice, knowledge, and skills in Geriatric Medicine / Frailty
  • Undertake history and physical examination to accurately assess the patient's condition.
  • Request, interpret and act upon relevant diagnostic tests and procedures.
  • Interpret, convey, and act upon complex and conflicting information to MDT members/patients/relatives including possible stressful situations, which may be delicate or offensive in nature (i.e., breaking bad news).
  • To identify patients in the Emergency department who are frail who could be cared for in a Virtual ward.
  • To work without borders with St Helens, Halton and Knowsley care providers
  • To directly liaise with Geriatricians and other Frailty specialists
  • To rotate throughout the Frailty services, including Ambulatory, Frailty Acute Inpatients, Frailty reablement ward, Urgent Community Response, Frailty Virtual Ward, and Community Frailty
  • Act as a clinical expert providing continuous support to nursing staff and other MDT members within and outside designated area of work.
  • Undertake advanced (invasive and noninvasive) procedures pertaining to patient need, in accordance with appropriate training and clinical supervision.
  • Independently prescribe and review medication in accordance with current legislation and trust policy.
