Band 6 Team Leader

  • 30 hours
Our complex care team is made up of
- the children's continuing care team, which provides respite in the home and school setting for children with complex needs.
- the Long Term Ventilation Team which provides packages of care in the home for children who require ventilation to assist their breathing.

  • Swanbourne gardens which is a 4 bedded respite unit that provides overnight respite for children with complex needs.
  • Palliative and end of life care.
As a team we strive to provide the children we support with a high standard of care.

As a band 6 team leader working within the team you would be mainly responsible for the delivery of care at Swanbourne Gardens
- our unique 4 bedded respite unit. Alongside this you may be required to assess, plan, implement and evaluate patient care in the child and families own home or in school / nursery. You would be at the forefront of teaching and assessing the competencies required for the staff to deliver day to day care for the children with complex health needs, this will include interventions such as care and management of tracheostomies, gastrostomies and ventilation support. You would be expected to provide supervision and appraisals for the junior staff and ensure all mandatory training is up to date. As a band 6 we would expect you to role model the trusts value and behaviour framework when working within the team.

To carry out the nursing process for complex children in the community. Including assessment, planning and implementing patient care.

To deputise for the Band 7 Deputy Matron based at Swanbourne Gardens.

To line manage junior staff.

To ensure all competencies are up to date for all staff to maintain safe delivery of care.

To complete the rota for staffing and children attending Swanbourne Gardens to ensure all commissioned packages are met.

If successful this vacancy will require an enhanced DBS check at a cost of £41.90. This cost will be deducted from your first Trust salary


In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £13.
