Description
We are looking to recruit a new Ward Discharge Facilitator to work on the Trauma Unit. The Trauma Unit is a 54 bedded Trauma and Orthopaedic unit based at the Royal Cornwall Hospital.


We are looking for pro-active, highly motivated individuals who can work autonomously and as part of a large multidisciplinary team.

The applicant will be expected to be enthusiastic, organised and have excellent communication and IT skills.

The role will focus on supporting flow of our trauma and orthopaedic patients to ensure all discharges are timely and meet the patients' needs by liaising with a number of members of the multi
- disciplinary teams.


To provide support in order to facilitate and drive the safe, effective and timely transfer of patient care from the Trust to the most appropriate setting to meet the needs of the patient.

To promote the philosophy of discharge planning as an integrated whole system approach which delivers an effective service.


Actively communicate with patients and their carers with the continuous process of discharge planning and ensure they are involved at every stage.

Assist in the co-ordination of patient discharge and support the development, implementation and evaluation of such processes.


Liaise with Registered Nurses, Doctors, Therapy Services, District nurses, Acute Care at Home and Social Services, CHC and OWC teams, Housing and any other appropriate members of the Multidisciplinary team to provide a proactive and continuous process of discharge planning.
