Remote working role with home visits

Hunter Mental Health have an exciting opportunity for a great CPN or Social Worker in an Older Adults team. The role will be providing treatment and care to patients aged 65 and over with a mental health illness including dementia in a Maidenheadbased service. This mental health trust strives to provide compassionate help and support to enable older people to live independently.

They will need someone with the following:

Experience working with Older Adults is desirable but not compulsory:

Ability to provide excellent treatment and care reviews to patients and their families/ carers

Ability to take responsibility for managing urgent clinical calls, including risk assessment and management

Ability to take responsibility for supporting team members in their decision making:

Good organisational skills:

Good Communicator:

Good computer skills
