The Senior Caseworker work as part of the Hammersmith and Fulham Navigator Service.

The role is part of an intensive floating support service, and is made up of 3 Senior Caseworkers, a Deputy Manager and a Service Manager.

The service aims to support individuals with complex support needs to find and sustain accommodation to end the cycle of homelessness.

As a Senior Caseworker, you will:

Work with a caseload of clients as well as supporting colleague's clients as appropriate.
Be part of a busy team environment working closely with those new to, or at risk of rough sleeping.
Provide a personalised case management service to clients, turning assessment recommendations into practical action plans which fully involves the client.
Build and maintain effective relationships to support clients throughout their recovery.
Develop a knowledge of relevant legislation such as, welfare benefit and housing legislation.

Case Workers work Monday-Friday, typical 9-5 but show flexibility in their hours in order to engage their client group.

This could include making use of TOIL to start earlier/finish later in order to attend an appointment outside of typical working hours.

About you

The Senior Case Worker will work peripatetically: within clients' hostels, support accommodation, on the street and within the community, providing a through-care model.

Some experience of working with vulnerable people with complex needs and supporting them to achieve and sustain positive outcomes would be beneficial.

However, above all we are looking for inspirational, committed individuals who have a genuine desire to support people to transform their lives.
