Primary care is the cornerstone of the NHS - providing holistic care to patients and serving the health needs of local communities.

Effective primary care is characterised by the strength of team working and ongoing relationships between patients, GPs, community services and other professionals.


As outlined in the NHS Long Term Plan, Primary Care Networks (PCNs) are support groups of practices to come together locally, in partnership with community services, social care, mental health and other providers of health and social care including the voluntary sector to enhance the offer to patients and ensure a whole system approach to primary care, with practices at the centre.


Job Summary
We are looking to recruit to the post of Health and Wellbeing Coach, to work within our Primary Care Network multidisciplinary healthcare team, providing 1:1 personalised support to people who are referred to them by team members


As a Health and Wellbeing Coach, you will have good communication, negotiation and people management skills and act with compassion and integrity.

You will have experience of using coaching approaches, frameworks and models or other helping strategies, for example, motivational interviewing.


This is a non-clinical role, but one that supports patients to address and tackle issues that are having an adverse effect on their mental and physical health such as housing, debt or social isolation, making lifestyle changes to promote an improvement with their wellbeing.

You will work with a diverse range of people from different cultural and social backgrounds. The ability to work confidently and effectively in a varied, and sometimes challenging environment is essential.


This is a vital role within the Primary Care Network and the post holder will provide health and wellbeing coaching for the most vulnerable people in the community, including the frail / elderly and those with other long-term conditions.

You will work closely with GP and practice teams to manage a caseload, ensuring appropriate support is available to people, families and carers.


You will work within our Wellbeing Team which consists of Health and Wellbeing Coaches / Social Prescribers and an Occupational Therapist to provide an all-encompassing approach to personalised care and enable people to work out how best to use the health and care system.


You support people to take pro-active steps to improve the way you manage their physical and mental health conditions, based on what matters to them.

You support people to develop their knowledge, skills and confidence - or to build their "patient activation" - in managing their health and care, to improve their health outcomes and quality of life.

Health coaches do this by coaching and motivating patients through multiple sessions to identify their needs, set goals, and supporting patients to achieve their personalised health and care plan objectives and providing interventions such as self-management education and peer support.


You will manage and prioritise a caseload, according to the needs, priorities and support required by individuals in the caseload.

You identify when a person's needs are beyond the scope of the health and wellbeing coach role and refer them back to other health professionals or organisations.

Health and Wellbeing Coaches may work with people by phone, by video conference, or face-to-face.

Interested? To be considered for this role, you will need:

  • Higher level of education (e.g., NVQ level 3 or A levels or equivalent)
  • Experience of using coaching approaches/frameworks and models or other helping strategies e.g. Motivational Interviewing
  • Experience of working directly in a community development context, adult health and social care, learning support or public health/health improvement (including unpaid work)
  • Experience of supporting people, their families and carers in a related role (including unpaid work)
  • Experience of supporting people with their mental health, either in a paid, unpaid or informal capacity
  • Experience of creating personcentred care/action plans.
