An excellent Support Provider in
Bristol are recruiting for a
Support Worker to join their team in supporting residents to access local healthcare, addiction, and mental health services.


Location:
Bristol


Contract duration:
Temporary to permanent


Hours:37 Monday to Friday 9am - 5pm


Rate:
£14.34 via Umbrella

Day to day responsibilities include:


  • Produce, implement and review support plans in partnership with service users in line with agreed policies and procedures
  • Liaise effectively with all agencies and other parties involved in service user's support in order to deliver the support package including referrals to other support providers
  • Identify, assess and manage risk to ensure risk is minimised
  • Build and sustain relationships with service users to enable delivery of the support plan and to enable them to make informed decisions about their mental wellbeing, housing and support, maximising their independence and providing advocacy role where appropriate
  • Contribute to initial assessments with guidance from Senior Worker
  • Promote and facilitate service user involvement and inclusion within the organisation and the community
  • Contribute to project work, development initiatives and implementation of policies and procedures with guidance from Team Manager and Senior Worker
  • Manage caseload, documentation and time effectively, making best use of supervision, training and staff development

Essential criteria include:


  • Proven experience of working with people with multiple needs (e.g. mental health, homelessness, alcohol/drug dependency, rough sleeping)
  • Enhanced DBS issued within the last 12 months or on the update service
