To work as part of a specialist children and young people's mental health team to provide intensive home based support and interventions for young people and their families.
To provide support to young people and families which promotes resilience and reduces the likelihood of requiring input from specialist mental health services in the future.
The post will support the children and families senior mental health practitioners in delivering safe and effective care to young people and their families, as agreed in their care plan.
The post will involve working under the supervision of a senior member of the team but will include independent work in the young person's home and other community settings.
The hours of operation for the Children and Young People's Mental Health Community Intensive Support Team will be variable and will require out of hours working that will be 7 days per week.
