Intermediate Care Social Worker


A Local Authority based in the Greater Manchester region is seeking an experienced Adults Social Worker to join their Intermediate Care services which works to support the safe discharge of patients from hospital into a safe environment where they can stillaccess an appropriate level of care.


Highlights:


  • Help support patients to be discharged safely from hospital into intermediate care where they can still be supported before returning back into the community
  • You will have the opportunity to work from home saving you time and money as you will only have to commute to the office a few times/week
  • You will have the opportunity to manage your diary and arrange visits as long as they fall within timescales.
  • The team is well established with most staff having been on the team for some time and a large proportion employed on a permanent basis
  • The pay rate for this post is up to £35/Hr which is higher than market rates for the region giving you the opportunity to boost your income on the run up to Xmas


This is an excellent opportunity for an experienced Social Worker to take on a post that offers a higher degree of flexibility as you will be able to work from home and manage your diary as you see fit as long as visits are completed within timescales.

Alongwith the greater autonomy you will be supported by an established team and long standing manager who is always accessible should you require further support.


The service aims to provide a safe discharge option for those patients that no longer require the level of care provided by a hospital but still require a care package to reduce risk of readmission.


To be considered for this post you should have experience of working within an Adults service in the capacity of a Social Worker.
