About the role


Second Step Recovery Navigators are committed to working with a recovery-based approach and supporting people to meet their individual needs.

You will build trusting and supportive relationships with clients to help them to engage with the NHS Physical Health Checks and help them to improve their physical health by accessing local services.


The people you will be contacting will be a patient at one of the participating surgeries and they will be entitled to an NHS Physical Health Check.

You will be contacting them by telephone to explain the purpose of the Physical Health Check and what's involved and book their appointment.

After they have received their Physical Health Check, you will be contacting them to support them to identify local services that may help them to improve their physical health, for example, a walking group or gym membership.


You will also support people to access Second Step's small grant funds that can be used to pay for gym memberships and exercise classes.

You will be part of a friendly team, with direct support from a Line Manager.

We are recruiting for two Recovery Navigator roles:

Role 1:


Recovery Navigator in Bristol - based at Second Step offices at Brunswick Square with travel to Montpelier Health Centre GP Surgery in Bristol, spending one whole shift at one location, you will not be required to change locations during the day. Some home working and community visits can be offered where appropriate.


Hours:37 hours a week (days and times are flexible depending on your preference, part-time or full-time within Monday - Friday 9am - 5pm, no weekends or bank holidays).


Fixed term:
until March 2024


Salary:
£23,037 - £25,182 FTE (pro rata for part-time)


All travel expenses are reimbursed

Role 2:


Recovery Navigator in North Somerset - Boulevard, Weston-super-Mare with travel within Weston, Worle and surrounding villages, spending one whole shift at one location, you will not be required to change locations during the day. Some home working and community visits can be offered where appropriate.


Hours:37 hour (days and times are flexible depending on your preference, part-time or full-time within Monday - Friday 9am - 5pm, no weekends or bank holidays).


Fixed term:
until March 2024.


Salary:
£23,037 - £25,182 FTE (pro rata for part-time)


All travel expenses are reimbursed.
