We have an exciting opportunity for a Band 4 Diabetes Primary Care Network (PCN) care and education Coordinator, to join our friendly team within Brent Community Diabetes Service.
The PCN care and education coordinator plays an important role within the PCN by adopting a flexible approach to patient care and service delivery.
The aim is to help patient to improve their quality of life.
The post holder will be responsible for the coordination, planning, implementation, and evaluation of the diabetes education as well as providing clinical support to patients.
The post holder will take responsibility for booking clinics, processing new referrals, updating waiting lists and communicating appointment related information to patients/carers.
And is expected to be able to act on their own initiative in dealing with day-to-day issues and the screening of correspondence, both written and verbal.

The post holder will be responsible for the coordination, planning, implementation, and evaluation of the diabetes education as well as providing clinical support to the patients.

Each network has a champion who is an executive director at the Trust and they hold regular meetings to discuss issues and make plans to improve CLCH
To work collaboratively with the GP's to plan, coordinate and provide diabetes education and clinical support to people with diabetes, their family/carers.
This will include support, advice and education in order to maximise the potential quality of life and independence at all ages.
Support patient use of technology and associated devices such as blood glucose meters including download and interpretation of clinical data.
To maintain a kind, caring and professional attitude towards patients, relatives and colleagues within the clinical setting, taking into consideration dignity, privacy, and confidentiality, cultural and religious needs.
