The Performance, Audit & Failsafe (PAF) service was established in London diabetic eye screening programmes (DESPs) were recommissioned into five super programmes in October making up North Central, North East, North West, South East and South West London DESPs.

Moorfields Eye Hospital NHS Foundation Trust is a primary referral site for a number of London DESPs.

The PAF service specialises in delivering failsafe services to both private and NHS care providers of diabetic eye screening (DES) in London and across the UK and is the leading hospital eye service (HES) provider for failsafe services to DES.

Triage all DES referrals from DESPs according to clinical feedback and national and local protocols and ensure no referrals are missed once received within the trust

Track and monitor DES referrals according to their triage status from screening to treatment and discharge and send update reports to DESPs

Update the DES referrals database

Register referrals and book appointments for patients referred to the City Road

Carry out call reminders to facilitate uptake of appointments and reduce non-attendance rates

Collaborate with admin and clinical colleagues to ensure all patients referred from DES are offered appointments, receive clinical care and treatment in accordance with the national DES guidance

Prepare DES data collated by the service for analysis and reporting

Track hospital cancellations and breaches which have the potential to lead to patient safety incidents

Participate in quality and safety audits to improve service delivery and drive excellence in patient care

PROBATIONARY PERIOD


During the probationary period, your suitability for continued employment will be assessed and provided a satisfactory standard is achieved and maintained, your employment will be confirmed.

The probationary period may be extended or your employment terminated either during or at the end of probation, in line with the trust Probationary Policy.

For the duration of the probation period, the notice period will be in line with contractual obligations on either side.


Ensure diabetic (DR) and non-diabetic (non-DR) referrals are accepted and declined according to the protocol for managing DR referrals i.e.

confirm patients are not already under the care of Moorfields for the condition referred and that they either have an appropriate follow up appointment or booked a first appointment within the timeframe in accordance with the referral type.

Ensure diabetic referrals for patients who are already receiving ophthalmological care at Moorfields at the time of the referral being received from DESP are appropriately sent for clinical review and managed according to clinical feedback and


Ensure patient details such as GP details, home address and contact numbers are up to date on PAS when processing referrals.

Register both DR and non-DR referrals for City Road and non-DR referrals for St. George's.


Upload DR and non-DR referrals for City Road onto CITO to enable access of referral by clinicians when patient attends clinic.

Book first appointments for patients with a DR referral to City Road.


Telephone patients referred for DR to notify them of their appointment to facilitate uptake of appointments and lower DNA rates.


Send daily and weekly referral receipts to DESPs and booking teams across all relevant Moorfields' satellite sites for confirmation of receipt of referrals.

Failsafe

  • Collaborate with the colleagues responsible for booking at City Road and satellite sites to ensure patients are booked appointments into the appropriate clinics within the national mandated timeframes for receiving hospital consultation, including subsequent appointments for patients who DNA or cancel appointments are managed in line with the trust Access policy.
  • Ensure the patient's GP is notified of nonattendance/nonresponse and discharge to facilitate and encourage attendance by checking the information provided in clinical outcome letters and letter templates used.
