Description

To provide a through psycho-social assessment of the client (aged 16+), this will include a risk assessment and a care planTo deliver individualised interventions based on individual care planLiaison with all referrers, members of the Primary Care Mental Health Service, Secondary Care Mental Health Services, and Voluntary or Statutory agencies as requiredTo manage own caseload by delivering brief interventions to the client, whilst maintaining and allocating new clients from the Primary Care Mental Health Practitioner waiting listTo deliver agreed clinical contacts as set by serviceTo use Information Technology to support the care process to benefit service users and their families through recording relevant data.

Record patient activity and interventions in a contemporaneous manner whilst acting on this information if necessaryTo work autonomously within professional guidelines and code of conduct,Pennine Care NHS Foundation Trust policies and procedures.

To be aware of and work towards the Trusts strategic goalsTo undertake any other reasonable duty, which is appropriate to the band, when requested by Senior Staff
