Description
Duties of the post.

The post holder will work with the hospital based palliative care team comprising of 1 WTE nurse consultant and 2.5 WTE Band 7 specialist palliative care nurses.

Close clinical links will be essential with neighbouring community teams. The post holder will have a broad training in palliative care, with experience in leading a multidisciplinary team.


They will be equipped to advise on a range of palliative care problems, supporting teams to develop their skills in palliating symptoms and supporting patients and families psychologically.

The post holder will be expected to be an advocate for patients with palliative care needs especially in managing transitions of care.
