Out of Hours Care Coordinator
Thornbury Community Services (part of Acacium Group)
Bristol, BS99 with hybrid working opportunities
Permanent, Full Time 42 hours a week
Shift pattern - 4 on 4 off, working 12 hour shifts (2 days 07:00-19:00 and 2 nights 19:00-07:00)
£24,500 per annum + bonus (Salary rises to £25,500 after 6 months)

- Ensure clients clinical and social needs are met by having selected and maintained the best nurses or carers for their needs
- Plan rotas and make decisions on nurses' suitability for particular shifts and availability
- Work with the clinical and remote teams to ensure all administration is in line with CQC requirements in a timely manner
