We are looking for a Mental Health Practitioner to join the King's Health Partner's (KHP) Homeless Team.

This is an exciting opportunity to be a part of an important and innovative service, one of only a few specific homeless mental health inpatient teams in the UK.

The role would suit someone with experience or interest in homelessness and inclusion health.

The role includes working with the multidisciplinary team (MDT) to support those that are homeless or vulnerably housed and admitted to a mental health ward within Lambeth or Southwark.

This is a diverse role which includes autonomous working and the opportunity to be a part of a dynamic team, led by evidenced based practice, linked with regional and national homeless research and strategies and a strong commitment to a personalised approach.


The post holder will support the team with managing a caseload of homeless inpatients, undertaking holistic and personalised assessments of people's needs, and working together with the individual, the ward and hospital discharge teams.

The post holder will use their knowledge, experience and communication skills to support and advocate.

Examples of this may be supporting patients with lost ID documents, signposting to specialist community services and accessing specialist legal advice.

The post holder will support patients with transitional work into the community. This may involve handing over to CMHT's, practical support, as well as an assessment of needs in the community.

The post holder will work within the MDT but will also be expected to work autonomously.

They will work collaboratively with external agencies such as the neighbouring local authority housing departments, as well internally such as with the hospital discharge teams.

They will have a role in developing and maintaining key relationships, such as with local authorities and 3rdsector organisations, through regular correspondence and meetings.

The post holder will work in partnership with the wider KHP homeless teams at King's College Hospital and St Thomas's Hospital, by attending regular meetings and sharing ideas and concerns.

There will be an expectation to support the team with delivering specialist training to the inpatient wards.


The Team is currently based at Lambeth hospital but will be moving to Maudsley hospital (along with the Lambeth acute wards) in spring of 2023.

Lambeth Hospital


Lambeth Hospital is based in south London, zone 2 and is close to a vibrant high street with lots of shops and restaurants.

It is within walking distance of Brixton (overland and tube stations), Clapham North tube station, Clapham High Street rail station and Wandsworth Road overland station, offering easy transport links to other parts of London.

Maudsley Hospital(headquarters)


Our Trust headquarters is located at Denmark Hill less than 5 minutes from the train station (zone 2) and is within walking distance from the beautiful green spaces of Ruskin park and the vibrant high-street that offers great shopping opportunities and with a wide range of restaurants.


  • To work autonomously, managing a caseload of patients. To work collaboratively with the multidisciplinary team, as well as colleagues within the trust and local authorities, providing an integrated approach to the care of homeless people with mental health needs.
  • To assess, diagnose, plan, implement and evaluate effective health and social interventions for inpatients experiencing homelessness.
  • To deliver individualised, specialised advice to service users, liaising with necessary support agencies / authorities to achieve the best possible outcome on discharge.
  • To have knowledge of the trimorbidity (physical health, mental health and substance use) and health inequalities that commonly impact on the homeless population.
  • To support service users to adopt personalised recovery strategies that promote their choice, wellbeing, healthy lifestyle, independence and self-care.
  • To promote the involvement of families and carers in patient care. Reflecting patients' diversity, equality and rights to ensure their needs are met.
  • To write necessary reports, case records and correspondence in line with Trust policy. To undertake all administrative duties, including data input into electronic recording systems.
  • To have an understanding of the laws and legislations that effect homeless people, such as The Homeless Reduction Act and Duty to Refer, as well as immigration laws and know how to support those with No Recourse to Public Funds. To keep up to date with changes in legislation, practice and policy.
  • To work collaboratively with local acute hospital homeless teams (King's College Hospital and St Thomas's Hospital), attending regular meetings, reflective practice and training sessions.
