We are recruiting for a Health Facilitator Nurse who wishes to join #TeamIOWNHS.

We are seeking an energetic, highly motivated, and experienced registered Learning Disability nurse who has extensive learning disability and mental health experience to work within our Community Learning Disability Team based on the Isle of Wight.

You will be expected to manage your caseload effectively to improve the experience for the adults who have a diagnosis of Learning Disability and /or autism.

The ability to communicate effectively and share information appropriately within a multi-agency context is also essential.

You will need to have excellent communication and interpersonal skills, be flexible and capable of working independently, as well as part of the wider Adult CLDT.

The Adult CLDT is Island wide, clinics/visits will be held in various locations across the Island, therefore the post holder must be a driver, due to the ongoing monitoring of COVID 19, it is essential that the post holder must also be able to use other platforms to complete assessment and reviews.

The post is full time Monday to Friday
