As part of Adult Social Care, Brokerage works with the Health and Social Care teams to improve outcomes for people, working closely with both internal and external partners.

This aims to support more people to live longer, healthier and independent lives.

Brokers work closely with health colleagues to support safe hospital discharges for all NYCC residents.
