We are excited to offer an opportunity for a B4 Associate Practitioner to join the Discharge Hub team at North Tees and Hartlepool NHS Foundation Trust.


The aim of the discharge and transport hubs is to promote patient flow, improve quality of care and patient experience for patients being discharged from the University Hospital of North Tees.


The post holder will be required to liaise with patients, families, carers, ward staff, internal departments and external agencies to ensure that patients being discharged via the hub experience a coordinated journey from ward to hub to home.


The post holder will be an accountable, professional, responsible member of the team and will deliver high quality, patient centered, compassionate care to patients being discharged via the discharge and transport hub.

The post holder will assume responsibility and accountability for patients to meet their needs in a safe caring environment that promotes dignity, privacy and respect.

To coordinate outstanding tasks to ensure patients are discharged safely and timely from hospital.

To provide support to the multi professional team by undertaking appropriate duties.

You will be expected to work flexibly and if required be available for both extended day and seven day working.
