An opportunity has arisen to work across an integrated dynamic, innovative and friendly team within Wirral Community NHS Foundation Trust Integrated Discharge Team (IDT).


The team is going through a very exciting period of transformation within the trust and an excellent opportunity has arisen for a position within the IDT team.

We are looking for an enthusiastic and forward-thinking Care Navigator to join our busy, pro-active team based at Arrowe Park Hospital.

**_Please Note. This role will be Transferring to Wirral Council during early/mid 2023 as part of the whole Adult Social Care service. At the point of Transfer the role will TUPE to Wirral Council who will become your new employer._
