The StaffBank for Epsom and St Helier NHS Trust are seeking an enthusiastic Community Nursing Matron to join the team at Surrey Downs Health & Care which is part of Epsom & St Helier Trust on a Bank/Flexible basis.
The role would be Based at Tattenham Health Centre with shift requirements of 2-3 days a week, 8:30-4:30pm - 7.5 hours a day.

Depending on previous relevant experience & previous NHS experience, higher banding/rates can be discussed.Main duties of the job:
The Community Matron will take a lead role to proactively identity vulnerable adults who have multiple and complex long-term conditions and who may be high intensity users of primary and secondary care services.

Detailed job description and main responsibilities:
Principal duties to include
Obtain information to inform the assessment of an individual
Establish the individual's functional capabilities in the context of long term conditions clinical management
Investigate and diagnose an unwell individual as part of the long term conditions clinical management
Plan, implement, monitor and review therapeutic interventions with individuals who have a long term condition and their carers
Assess individual's needs and preferences
Enable individuals with long term conditions to manage their medicines
Plan, implement, monitor and review individualised care plans with individuals who have long term care conditions and their carers
Co-ordinate and review the delivery of care plans to meet the needs of individuals with long term conditions
Develop risk management plans to support individuals' independence and daily living within their home
Use and develop methods and systems to communicate, record and report Present individuals needs and preferences
Procure services for individuals Manage the use of physical resources Support the protection of individuals, key people and others
Develop practices which promote choice, well-being and protection of all individuals Assess the health care needs of individuals with long term conditions and agree care plans
Enable individuals with long term conditions to make informed choices concerning their health and well-being Support individuals to live at home
