The National Health Service (NHS) has a number of sub-acute services that have developed in recent years from the guidance of NHS white papers such as the NHS Long term plan and NHS five year forward view (2014), which recommend for NHS trusts to deliver more care and services closer to a person's home rather than in acute hospitals.


As pressure on emergency departments across the UK grows, new care models will be pivotal in delivering and relieving some of these pressures by diverting work away from the hospital and into the community through the 2-hour Urgent Community Response, OPAT service and virtual ward offers.


In Cheshire West, the 'Hospital at Home' service will expand its offer of with the implementation of virtual wards allowing patients to get the care they need at home safely and conveniently, rather than being in hospital.

The NHS is increasingly introducing virtual wards to support people at the place they call home, including care homes.


The Hospital at Home workforce fits the NHS model well in supporting hospital level care at home; experienced General Practitioners, advanced clinical practitioners with prescribing rights, registered therapists / nurses and support workers offer a 7-day service that runs from 8am-8pm.

Allowing for wrap around care and an excellent patient experience as evidenced in the services monthly feedback from patients.


To provide high quality, efficient and comprehensive support to the Clinical Lead, Team Leader and Operations Manager for Hospital at Home.

To undertake a full range of responsibilities associated with the post using discretion, initiative and maintaining confidentiality.

To undertake a coordinated workload, to work as part of a team and be motived and able to work largely unsupervised.

To provide an effective, high quality administrative support service to the Division, specifically Hospital at Home.

To provide a comprehensive, efficient service on all aspects of administration including national data reporting requirements, diary management, document production and procurement service for the department.


The Countess of Chester Hospital NHS Foundation Trust consists of a 600 bedded large District General Hospital, which provides its services on the Countess of Chester Health Park, and a 64 bedded Intermediate Care Service at Ellesmere Port Hospital.


The Trust provides services in all of the surgical and medical specialties common to a District General Hospital with some specialist services.

The Trust serves the whole of the local population currently in excess of 250,000 residents which geographically covers populations in England and North Wales.
