An exciting opportunity has arisen in the Integrated Discharge Team for a full-time/part time Patient Flow Coordinator.

Participate in transferring patients to the Discharge Lounge, as early as possible, and throughout the day.

  • Take part in the wards board rounds and pick up any delays / escalations to act on.
  • Manage and support the home today list on Nerve Centre and ensuring patients are discharged as planned.
  • Communicate and close working with support services, Transport, pharmacy, discharge lounge, IDT, Red to Green team, Site Office and Ward staff as required.
  • Participate and support full capacity protocol when activated
  • Support the wards with booking transport 24 hours in advance to facilitate timely discharges.
  • Ensure all patients transferred to Discharge Lounge unless meet exclusion criteria
  • Ensure patient/relatives/carers are aware of discharge plans promoting positive patient experience using the SAFER and red to green processes.
  • Fast track drugs and progress chase directly with pharmacy
  • Ensuring Discharge transfer letters completed by ward Doctors using the Lorenzo system.
  • Attend bed meetings to support the integrated discharge team, the site team and Matron
  • Any other duties that support the work of the integrated discharge team

  • Contribute to the development of own knowledge and skills
  • Take part in learning activities, training and development within the trust, for own personal development and role development.
  • Maintain competence in statutory and mandatory training requirements through attendance and/or undertaking trainin
