The primary objective of this post is to ensure safe care is commissioned for the client group which we serve
The primary function of the role is to coordinate and lead the process of securing appropriate placements and support for eligible Continuing Health Care patients, Children's Continuing Care patients and all other cohorts in need of funded care and / or support, including those with Personal Health budgets via direct payment or Notional.
This will include building effective relationships with relevant professionals and key stakeholders e.g., patient's, relatives, assessors, PHB brokers, local authority representatives and placement providers etc.

- Identify availability of care provision and resources appropriate to meet patient needs. The actual clinical needs and care requirement will be determined by a nurse assessor
- Ensure the identified service is of the highest quality and relevant contracts are in place (in liaison with the contracting team) and meets the clinical requirements of the nurse assessor
