Description

This is an exciting opportunity for an experienced Speech and Language Therapist with special interest and experience in working with people with Learning Disabilities and mental health conditions to shape and contribute to the development of an outreach service.

You will form part of an MDT providing outreach support to patients referred to, or moving on from the High Secure Learning Disabilities Service, supporting implementation of national drivers such as the Transforming Care agenda and reducing restrictive practice.

The overall aim of the outreach service will be to improve the quality of care for people with a learning disability and autism, to improve quality of life for people with a learning disability and autism, to reduce inappropriate admission to high secure hospital care and support a successful journey on from high secure care.

You will be responsible for contributing to the identification of speech, language and communication needs (SLCN) of individuals, highlighting the reasonable adjustments to be made through formulation, treatment planning and risk management.

You will highlight environmental challenges and training needs for staff teams caring for patients with SLCN and/or dysphagia needs on the caseload and identify reasonable suggestions for teams to address these.
