Social Care Practitioner - Community Team
Looking for a rewarding career in Community Social Care with Adults?

We are looking for energetic, imaginative and flexible people to work in Adult Social Care. We have teams in community and hospital settings. You will undertake assessments of need and reviews of care packages and placements in residential homes.

You should have a qualification in health and/or social care. Social Care Practitioners will need NVQ Level 2 or equivalent. You must have experience of working with older people and people with a physical disability.

Working alongside social workers, occupational therapists and other health and social care colleagues you will be given great support, formal supervision and have opportunities for further training and development.
