Description

The postholder will undertake the following duties and responsibilities at all times demonstrating the competences assigned to the role: Communicates with service-users and carers, all members of the multi-disciplinary team and staff in external agencies using a variety of methods including Information Technology.

Regularly reports and effectively communicates service-users levels of function and interventions care records and verbal reports to Registered Occupational Therapy staff members and other members of the multi-disciplinary team.

Relates appropriately with service-users, carers, colleagues and others at all times maintaining professional boundaries.

Deliver agreed therapeutic interventions, in accordance with the care plan, to assist the service user to achieve their recovery goals including; improving activities of daily living, social inclusion, meaningful occupation, accessing employment, mental health promotion and physical health promotion.

At all times ensures that own actions, support the equality, diversity, rights and responsibilities of individuals. Identifies, minimizes and reports risks of danger, harm and abuse to senior colleagues. At all times minimizes risks to service user, self and others by undertaking, safe working practices. At all times works within agreed and established Trust policy and operational procedures.

Works with other Trust services to promote seamless service-user focused care, admission, transfer and discharge on identified care pathways. Responsible for the safe usage and cleaning of both equipment, maintaining stock control and handling cash as required.

Responsible for contribution towards planning and undertaking delivery of planned OT interventions with an identified group of service users Responsible for regularly working with service-users to negotiate and review existing plans to address identified needs under the supervision of a registered OT.

Works in partnership with service-users to assist them in identifying activities that are meaningful to them and that support agreed health outcomes.


Regularly supports service-users to access a range of meaningful activities and services to maintain their independence and promote their recovery.

Regularly provides advice and guidance to Service-users promoting healthy activities and lifestyle choices. Regularly support Occupational Therapists and Service-users during interventions to meet their physical and mental wellbeing. Promotes Service-users independence in meeting their recovery needs.

Regularly provides information, support and guidance to Relatives, Carers, and family in line with service user wishes and confidentiality.

Reports concerns about all staff members work performance to registered Occupational Therapist or Operational Lead At all times takes responsibility for own personal and professional development and supports the development of colleagues, students, trainees and volunteers.

Regularly accesses and makes use of supervision and provides supervision to other staff members. Regularly participates in activities focused on the delivery of a high quality customer-focused service.

Regularly undertakes activities to (and makes suggestions about how to) improve services.
