Qualified Social Worker - Tameside MBC, Greater Manchester - Adults Services - Up to £35ph
We are looking for 4 social workers to initially complete reviews in the community, there after there maybe opportunities to work also within the hospital discharge team to support with flow and safe discharges.
The role is customer facing so there is an expectation to work in the office / visit service users in their own homes or within residential settings ( in the community ) Essential care driver is a must and experience of fast paced work, completing Care Actassessments, reviewing reablement plans to assess for longer term care, ensuring that adults meet the eligibility criteria.
