Description

The post holder will demonstrate a high level of expertise and clinical leadership within the county wide Community Diabetes Service providing advice, education and support to staff, patients, their families and carers.

The post holder will be practising autonomously as an Advanced Practitioner within the service to provide patient-centred clinical care.

This will encompass the skills of advanced clinical assessment, examination, diagnosis and treatment within an agreed scope of practice throughout the service/ care group.


This is underpinned by a master's level award or equivalent that encompasses the four pillars of clinical practice, leadership and management, education and research, with demonstration of core capabilities and area specific clinical competence.


  • Support new ways of working that emphasises a more efficient and patient focused service, manages undifferentiated, undiagnosed conditions and will ensure the safe treatment, referral and discharge of patients with complex health.
  • To play a pivotal role in the operational development of the service. You will be involved in the strategic planning & development of Long Term Conditions.
  • To have a responsibility for leading the ongoing development of clinical practice & standards of care within the service, including the development of policies, procedures, protocols and guidelines in collaboration with MDT colleagues.
  • To critically engage in research activity, actively identifying gaps in evidence base and be instrumental in research implementation into clinical practice.
  • To have a responsibility to promote clinical excellence in the care of patients by providing clinical advice and support to other health and social care professionals. Their work will be underpinned by The Living Well model and partnership working with primary and secondary care as part of a system wide approach to care.
  • To work independently/autonomously and be accountable for their own area of practice and be compliant with their respective code of practice. They will act on professional judgement about when to seek help, demonstrate critical reflection on own practice, selfawareness, emotional intelligence and openness to change.
  • Actively participates and contributes to the clinical governance agenda.
