**Permanent contract | £11.59 to £12.46 per hour (Scale E) | 36 hours per week | Reed House, Norwich

Norfolk and Waveney are on an ambitious journey to embed Discharge to Assess as part of our Ageing Well Accelerator programme to ensure people being discharged from hospitals across Norfolk are supported to return home, access reablement opportunities and plan their ongoing care and support needs once discharged.

To support this journey, an exciting new Virtual Ward is being set up.

A remote service that will help patients to manage their health and care at home and we need Senior Business Officers to be the engine room to underpin this process.

You will provide a range of administrative and support services to The Virtual Ward which will allow patients to get hospital-level care at home safely and in familiar surroundings, helping speed up their recovery while freeing up hospital beds for patients that need them most.

The team is operational 7 days a week from 8am to 8pm.

We need a team member to work 12-hour days and are looking for a Senior Business Officer to provide this cover.

Some weekend working is essential and will be required.

The working patterns we are offering are:
- **3 x 12 hour days (1 post)

The role will also assist the work within the HomeFirst Hub to support admission avoidance for North and South Norfolk and Norwich.

You will be a team player with great working knowledge and experience of using Teams, Excel, data entry and Microsoft outlook.

You must be able to multitask including word processing notes whilst taking the information over the phone. You must be flexible, have a positive attitude and respect for sensitive data. You will have the ability to work at pace.

Tasks include taking telephone referrals from health professionals, updating information onto the relevant data bases, maintaining accurate data for analysis and operate in accordance with NCC and departmental practice and guidance.

You will have good listening and analytical skills, work with accuracy and great attention to details, be flexible and adaptable to new ways of working and be committed to supporting people to leave hospital safely.
