Hywel Dda University Health Board are currently looking to recruit a suitably qualified and motivated individual who has an interest in working with older adults experiencing mental illness both functional and organic to join the South Carmarthenshire Older Adults Community Mental Health Team based at Caebryn Unit, Prince Philip Hospital, Llanelli.


The post holder will be responsible for the assessment of care needs, the development of programmes of care and the implementation and evaluation of these programmes of care for a defined clinical caseload.

The post holders will also be expected to undertake the role of a Care Co-ordinator within the Care Treatment Planning under the Mental Health (Wales) Measure 2010.

Essential to the post is the ability to work in collaboration with the multi-disciplinary team and with voluntary organisations, local authority and other appropriate health and social care sectors to negotiate care provision across the primary/secondary care interface.


The individuals will be expected to respond to crisis situations, exercising clinical judgements and assessing complicated events and situations within clients own home, in other care settings and as part of their duty rota involvement.


As an autonomous practitioner, you will demonstrate self-management skills and independence in thought and action and take personal responsibility for professional performance e.g.

regular lone working and with clients in the community.


The post holder will be responsible for supervision, PADR's and caseload management support of the Health Care Support Workers within the CMHT.

The post holder would be expected to deputise for the band 7's in their very short term absences.

  • Seven community hospitals: Amman Valley and Llandovery in Carmarthenshire; Tregaron, Aberaeron and Cardigan in Ceredigion; and Tenby and South Pembrokeshire Hospital Health and Social Care Resource Centre in Pembrokeshire;
  • 48 general practices (four of which are managed practices), 47 dental practices (including three orthodontic), 99 community pharmacies, 44 general ophthalmic practices (43 providing Eye Health Examination Wales and 34 low vision services) and 17 domiciliary only providers and 11 health centres;
  • Numerous locations providing mental health and learning disabilities services;
  • Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.
