Description

The post holder will be required to work on their own initiative and demonstrate a willingness to help others and be a key member of the team, with responsibility for specifically allocated tasks.

The post holder will be expected to follow set processes, prioritise their workload and demonstrate initiative in order to fulfil the requirements of the post.

Excellent interpersonal and communication skills are essential plus an ability to deal with sensitive and confidential phone calls.

The post holder will be required to be proficient in word processing and inputting into databases, in addition to diary management.

To ensure all NHS placement contracts are in situ and agreed within the pricing structureTo support the brokerage of care for those CHC patients who have been deemed eligible individuals, following guidance on clinical needs, and in doing so deliver appropriately commissioned care placements and packages of care.
