About the Role:


We have an exciting, new opportunity for a Care Co-ordinator to join our Primary Care Network, supporting our 5 GP practices across the Braintree area to assist in re-shaping primary care across our communities.

The role is for 15 hours per week and involves working very closely with the practices and the multidisciplinary team (MDT) within the PCN.


If you are a skilled Care Co-ordinator with excellent person-centred planning experience, with local knowledge of key support and advice and support to our patient population, then we'd love to hear from you.


We are looking for:

  • Experience of working within healthcare or the voluntary / community sector, supporting vulnerable groups
  • Ideally a Diploma Level 2 in Health and Social Care or equivalent qualification
  • Strong listening and communication skills
  • Familiarity with local resources and services and how to access them
  • Someone who is motivated by helping people, with care and empathy in delivering personcentred support in a nonjudgmental way
  • Experience of completing holistic personcentred care planning assessments
