Description

To work within the Memory Assessment team and within the wider Older peoples services providing support to individuals living with Dementia and their families/carers.

To monitor current medication regimes and Signpost to relevant services and highlight any unmet needs. To help service users access appropriate resources.

To support educational/therapeutic groups provided my the Memory Service an on occasions by the wider community.


To provide support to people living with dementia and their families to help them achieve the best quality of life possible.

To assist qualified practitioners in the assessment and review of patients and the monitoring of the effects/side effects of medication

Positively promote independent living of service users within the community

To monitor the service users level of functioning and cognition and reporting any improvement/decline to qualified practitioners

The post holder must ensure that their behaviour and interests inside and outside of work do not conflict with their trusts position, duties and responsibilities

The post holder must carry out all duties and responsibilities of the post in accordance with the Trusts Equal Opportunities and Equality and Diversity policy


To work as part of the Memory Assessment team, providing support to individuals with a diagnosed dementia, assisting qualified practitioners in assessing ,reviewing and monitoring individuals, cognition, functioning ,medication, and support network.

To provide support for carers and help people to access the necessary resources which may enable them to live safely and well.
