The Cancer Patient Pathway Navigator will play a crucial role in the proactive management of patients referred onto the pathway.

The post holder will work within radiology and collaboratively with the wider multidisciplinary team (MDT) to ensure patients receive comprehensive and timely investigation of a suspected cancer.

They will be expected to proactively break down any barriers to access and provide a key point of contact for patients.

The post holder will work with the wider MDT to develop processes conducive to meeting timed milestones.

Detailed job description and main responsibilities


The post holder will work as part of the radiology team, and will be responsible for monitoring patients' progress along their cancer pathway on a daily basis, highlighting problems to the management team and escalating where necessary according to local protocols.

The post holder should engage with continuing professional development to gain and maintain a full understanding of the national cancer wait time targets.

Build and maintain effective working relationships with the wider MDT in pursuit of delivery of care within defined cancer wait time targets.

The post holder will be required to maintain cancer referrals with the use of Radiology systems. Liaise with the relevant Radiology Modality Manager to escalate patients in need of imaging. Regularly review all 2 week wait patients within Radiology and identify delays in managing patients.


The post holder will ensure close liaison with the multi-disciplinary team in Radiology as well as wards, GP surgeries, outpatient clinics, patients and nursing medical/administrative staff in order to provide a prompt patient focused service and maintain good relations with internal and external referrers into the service.

This will also include attending the Cancer PTL meetings.


The post holder should possess the ability to use his/her initiative, working without direct supervision, organising and taking control of specified duties, escalating where necessary to the Deputy Service Administration Manager.

It is essential that the post holder is aware of the need for flexible working arrangements. They may be required to work at any of the NUH Campuses, including the Treatment Centre.

Qualifications:

Essential criteria

  • A to C grade GCSE's or equivalent including English and Maths
  • Experience in using Microsoft Word, Excel, Outlook
Desirable criteria

  • Competent at using Trust IT systems, CRIS, Careflow, Notis
Experience

Essential criteria

  • Recent experience in an administration/clerical role
  • Ability to work in a pressured environment
  • Must have experience of liaising with different staff groups
Desirable criteria

  • Previous experience of managing own workload
  • Knowledge of NUH policies and processes
  • Knowledge of the pathway targets and diagnostic imaging targets
