Description
The role of a Project Worker in the Deptford area is to support young people in becoming independent and transitioning out of the pathway into independent accommodation.
You will be responsible for various tasks to achieve this goal.The key responsibilities include:
1.
Caseload Management:
- Hold a caseload of approximately 6 young people.
- Conduct regular key working sessions to develop SMART (Specific, Measurable, Achievable, Relevant, Time-bound) Support Plans

Needs and Risk Assessments:
- Complete comprehensive needs and risk assessments
- Focus on identifying suitable move-on accommodation options
1.
Referral Assessment:
- Interview and assess referrals to the project
- Ensure they meet the criteria and accept suitable young people
1.
New Resident Integration:
- Help new residents feel comfortable and provide information about the building and local services
1.
Mediation and Reintegration:
- Collaborate with mediation or other social workers to facilitate young people's return home if appropriate
1. Move-on Support:
- Assist young people in identifying opportunities for appropriate move-on accommodation (e.g., private rented sector, supported housing, shared or self-contained)
- Make relevant referrals
1.
Independence Preparation:
- Work with young people to identify activities that prepare them for independence and help them progress
