We are excited to share that we are looking for more enthusiastic, inspiring, Living Well Facilitators as we continue to build our service.

These roles will be working within Living Well Care as part of Perth & Kinross Health and Social Care Partnership.

Living Well Care is a new and innovative approach to care at home and your role will be to improve a person's quality of life through delivering person centred care, as well as working with front line statutory workers such as Social Workers, District Nurses, Occupational Therapists, CPNs and GPs to identify people on their caseloads suitable for community-based supports and signpost them where appropriate.

An important part of the role will be making time to build strong relationships with our service users, their families, the communities they live in, other service professionals involved in their lives and the wide range of services within our communities in order to support the whole person.


Living Well Care is a Care at Home Service, working over 7 days a week, meaning there is a requirement to work flexibly, including evenings and weekends.

There are a range of contracted hours available and some real flexibility around shift patterns.

The role will help people to remain at home through providing direct personal care support whilst also assisting individuals to access community-based supports in Perth and Kinross.

This will help improve outcomes for people and also minimise the number of professionals involved in someone's life.


This post will require you to demonstrate flexibility and a broad range of skills to respond to the changing needs of individuals.

Ideally, you will be able to demonstrate experience of multi-disciplinary working and be committed to working closely with colleagues from other disciplines.

You must be able to work effectively within a small interdependent self-managing team, where the team culture provides each member with the opportunity to be autonomous; responsible for raising awareness of the team's ethos and reflecting on practice to help improve service provision through innovation and where collaborative and leadership qualities are at the heart of our practice.

You must be willing to learn aspects of many different roles including but not limited to, carrying out reviews, undertaking risk assessments and learning about technology and equipment that might assist service users.

You will have the ability to build trusting and productive relationships with people. You will work well with service users to promote community-based services and other supports as appropriate.

You will be confident and become a main point of contact for the team within the locality and an ambassador for Care at Home Services.

A current driving licence and access to personal transport is essential. Teams are based in Blairgowrie and Crieff.
