Description

The Integration Lead for "Homeward Assessment Team" will provide leadership across two multidisciplinary teams that are in the process of integration.

The teams are based within emergency and short stay departments at Gloucestershire Hospitals NHS FT. The primary purpose of the role would be to leadership and accountability around the Integration efforts.

Context

The Frailty Assessment Service and the Integrated Assessment Team have integrated under the "People's Framework" arrangements. Their new joint name is the "Homeward Assessment Team" (HAT).

The Frailty Assessment Service is a GHFT employed team operating in both ED and the Frailty Assessment Unit at the GHFT.

The Integrated Assessment Team is a GHC employed team, also based in ED.

The vision of the integrated team is one team with a focus on ''improving patient experience through admission avoidance with high quality, early, specialist assessment and short stay patient management competences.
