An exciting opportunity has arisen for a Team Leader post based within the supportive and enthusiastic South Neighbourhood.

We are an integrated neighbourhood care team consisting of Community Therapists, Community Matron's, Care Coordinators, Health and Wellbeing Support workers, Community Nursing and Neighbourhood assistants.

We pride ourselves on our relationships within the neighbourhood and with other statutory and voluntary agencies.

You will be supported and encouraged to use all your skills to allow yourself, the team and your patients to flourish.

As the South Neighbourhood Team Leader you will work closely with the Clinical Lead to help move forward any new developments within the neighbourhood care model


We will support you through regular supervision and the Trust appraisal process, which will help you to fulfil objectives and opportunities related to the post.


We are looking for an experienced professional with a Specialist Community Practitioner qualification or Therapist with relevant leadership qualifications who has a real enthusiasm for caring for patients in their own homes.

You will be required to lead a team within South Neighbourhood and manage the day to day, on-going delivery and development of the team.


The applicant will support the leadership of the service within the structure and will provide a service that is responsive, high quality, supporting practice delivery and continually improving standards within the Clinical Governance agenda.


You will work with clinical and professional leads to implement best practice and ensure the maximum utilisation of resources to benefit our patients.

You will be a role model for junior staff, you will be responsible for moving the team into new models of care working with colleagues within your neighbourhood.
