Community Living are currently recruiting for experienced Support Workers to join our team and work in our Supported Living service, Cheam House.


Cheam House provides supported accommodation to adults with autism and learning disabilities and is located in the heart of Cheam in Sutton, Surrey.

All inidividuals have their own individual bedroom with en-suite shower room and share communal areas of the building.


Purpose


In this role you are a frontline member of staff, who is appointed to work in a particular service setting, though may be asked to undertake duties at other settings as part of planned transitions or assessments.


Key Responsibilities
Ø To provide care for Service Users' as planned in individual support plans

Ø To observe report and record changes in the Service Users' physical and mental health, as well as changes in behaviour

Ø To provide the necessary physical care to Service Users' to help them complete daily living activities, i.e. personal hygiene, clothing, laundry, maintaining personal environment, maintaining rest and sleep and activity

Ø To act in a manner to respect the customs, individuality, values, sexuality and spiritual beliefs and providing support to enable these to be followed

Ø To provide support in social activities, emotional support and stability to Service Users' to ensure effective engagement whilst maintaining professional boundaries

Ø To create and maintain professional supportive relationships with all members of staff, with other professionals and agencies

Ø To provide support to enable Service Users' to access Community based activities

Ø Ensuring team members are equipped to deliver a quality service to Service Users

Ø Contributing and supporting the continuing development of the service

Ø Actively participate in the transition of new Service Users


Communication
Ø To record all contacts with Service Users and continuous records

Ø To ensure effective communication with Service Users, relatives/carers, visitors, staff and others

Ø To ensure that all communication is within boundaries of Service User confidentiality and to seek guidance when unsure

Ø Exchange factual information with Service Users using persuasion, tact, reassurance, empathy and acknowledgement of the barriers to understanding

Ø To ensure that any information related to Service User or other personal safety is communicated immediately to other team members


Quality
Ø To assist in the collection of data for the purpose of audit, research and service performance

Ø To lead on the maintenance and monitoring of service standards

Ø To promote Service User involvement in all aspects of their care

Ø To contribute and make suggestions to the improvement of services

Ø To identify issues of concern at work, and acting to rectify these when needed

Ø To present a positive role modelling image to team members and the organisation

Ø To act as an effective team member


Risk Assessment
Ø To have knowledge of risk assessment and management tools currently in use within the Service and to provide support for Service Users in line with these

Ø To recognise potential crisis situations and act responsively and responsibly until assistance arrives

Ø To be aware of the potential for vulnerability and abuse amongst the Service User/Families and using the local policy to act in an appropriate manner in line with POVA

Ø To follow the Community Living Ltd Lone Working policy, ensuring that any autonomous activity is effectively reported and logged

Ø To take all possible precautions to safeguard the welfare and safety or staff, Service Users, visitors and the public, by implementing all policies related to health, safety and risk


Professional/Person Development
Ø To participate in supervision, appraisal and performance development

Ø To identify appropriate training needs in line with professional development

Ø To ensure attendance at all training, and training is kept up to date and compliant at all times

Ø To complete and work on agreed actions within Personal Development Plan


Key Result Areas
Ø To provide physical and psychological support to Service Users in accordance with the specified support plan/s

Ø To carry out duties as designated by the Service Manager

Ø To accurately record Service User information

Ø To provide support to staff, relatives/carers

Ø To attain and maintain KPI's set out by Service Manager

Ø To work proactively in accordance with the principles of social inclusion and promoting independence.

£11.00 per hour
