Frailty MDT/ Case Manager Support Worker (Barnet Planned Care

  • Frailty Service
  • CLCH NHS Trust), Permanent-Full time position.


The Ageing well funded service will provide proactive patient centred assessment, treatment, and care to minimise unplanned care hospital admissions and optimise patient's health conditions to improve quality of life.

The post holder will work closely with the core members of the multidisciplinary team and case managers.

You will provide information, advice and support to a caseload of frailty patients and their families and carers residing in Barnet.


Participate in the weekly MDT meeting and will be carrying out specific therapy/ nursing interventions and be responsible for collecting a range of information on clients' needs and health status.

Under the guidance and supervision of qualified therapists, allied health professionals, nurses, consultants, the service aims to identify and address the complex needs of frailty patients and carers across the pathway, by providing a range of innovative support solutions.

Other duties such as clerical and administrative tasks will be as required.


We offer regular clinical and managerial supervision, guidance and training for personal and professional development in a friendly and supportive environment.

This includes a program of In-service training and other training and development opportunities (the post holder will need to become a Trusted Assessor).

Part of the role may include recruitment, training and managing volunteers, to support service users and the delivery of the service.

  • The Frailty MDT Support Worker will play a key role in the Frailty MDT service, monitoring progress and facilitating agreed actions delegated by clinicians.
  • To participate as a general therapy assistant within the Frailty MDT under the guidance and supervision of qualified therapists and nurses, provide advice, information, support and signposting to secondary services and other organisations offering support.
  • To independently manage a caseload of noncomplex cases in consultation with and as delegated by Frailty Clinician's (Nurses, Physiotherapist Dietician, Speech and Language Therapist, and Occupational Therapy staff).
  • To be actively involved in carrying out specific Frailty Nurse, Physiotherapist Dietician, Speech and Language Therapist, and Occupational Therapy interventions with clients independently as delegated by these clinicians.
