The Homeless Intermediate Care Team (HICT) is looking to recruit an Administration Coordinator/Case worker to work in wider Integrated Care Operations Team.

The HICT provide support to people who are experiencing homelessness after their discharge from hospital.

The team works across North Central London and help clients in their journey from hospital to move on from homelessness for approximately 6 weeks post discharge.

They provide case management to clinical and social care act assessments to aid recovery both physically and mentally to coordinate the resolution of social, health, and housing issues.

The role will have two main functions. One being to provide administrative support to the team, through booking appointments, managing the team's patient management system, etc. The second will be to assist the team in attending appointments, etc. with clients. These will hugely vary in nature but commonly include embassy appointments and GP appointments.

The post holder will have a degree of autonomy and a proactive approach will be needed to prioritise effectively.

The post holder will have key working relationships with all members of the administrative and clinical teams including allied health professionals, support workers, and liaison with nursing and medical colleagues.

The post holder will be responsible for escalating and troubleshooting issues will contribute to finding proactive solutions to routine issues.

The post holder will need to have the ability to manage a busy and varied workload. Excellent communication (written and verbal) and strong interpersonal skills are essential to be successful in the role.
