We are looking to recruit an experienced General Administrator to join the Administration team within Community Frailty Service.

The Community Frailty Service launched in 2015 and operates across the Fylde Coast providing integrated care for patients over 18 with any physical long-term condition.


As the Community Frailty Service is a relatively new service, tasks and duties will evolve as the service further develops and it is important the post holder is able to work flexibly, be adaptable to change and at times use their own initiative to complete tasks.


The post holder will work as part of a Team to provide a high quality secretarial, administration and information service and act as a central reference point to all staff within the service including clinicians/managerial staff, Trust stakeholders and Community Frailty Service patients.

The role comprises of a wide range of duties and requires the ability to use multiple computer systems, manage and prioritise own workloads without direct supervision and have strong communication and organisational skills.


The admin hub is the first point of contact from patients, relatives, and other healthcare professionals and receives up to 250 calls per week.

In addition to providing day to day administrative support, the team also provide comprehensive support to the Senior Management team.

The post holder will (please also read the full job description & person specification)

  • Provide a comprehensive, administrative and information service on behalf of the Community Frailty Service to both internal and external stakeholders and the general public.
  • Adding and processing of new referrals and dealing with correspondencereceived and respond where required.
  • Effectively manage the Microsoft Outlook diaries of the Senior Clinical team and prioritising the competing needs of the Service.
  • Be responsible for telephone calls from both internal and external sourcesthrough the team hub, taking appropriate action to ensure all messages are received by relevant members of staff redirecting as necessary.
  • Where necessary, adding patients to the Telephone Triage list, documentinginformation accurately and escalating any concerns appropriately.
  • Booking appointments for new patients and follow up appointments withinagreed timescale
  • Ensure correct processes are followed to enable the service to maintainaccurate statistical records and be able to provide reports as requested
  • To arrange meetings as directed, liaising with attendees to confirmarrangements made, making roombookings and distributing agenda's. Minute meetings accurately and forward agreed.
  • Comprehensively use hospital information systems and EMIS to ensure thatpatient details are correct.
