The role will be based in Kings College Hospital, The Well Centre in Herne Hill and a lot of outreach contacts with young people across South East London.

with a possibility to extend if funding extended


Hours of work:37.5 hours a week**, to be worked with a willingness to be flexible. This post includes regular evening work.


External contacts:
Young people and their families; Voluntary sector; Social Care teams; Education Authorities and schools, Gyms, Local Exercise providers


Salary:

Band 5**The Well Centre is an innovative Youth Health Centre and provides access to a range of health, information and advice services for young people aged between 11 and 20 years.

We want the young people who come to us to be 'healthy, happy and safe'. We run sessions with a Health and Well-being Practitioner, Counsellor and Doctor on site or remotely.

We work on a holistic model of health in partnership with local agencies to ensure that young people's needs are met.


The Health & Wellbeing Practitioner with special interest in Weight Management will connect young people in to a broad range of services and interventions in their local area in a timely and effective way, led by the wants and needs of the young person's themselves and will specifically support young people accepted onto the Clinics for Excess Weight (CEW) project.


The post holder's responsibilities will include signposting, advising on benefits that are available such as financial advice, support and access to other services.


The post holder will be responsible for developing a directory of local services to be used as a resource by the team and the families.

They will also be responsible for co-delivering group interventions for parents and young people.

The support will be provided in the home, community and hospital settings.

The post holder will work collaboratively with the family, acute teams and any professional or voluntary services and will be required to develop their knowledge about obesity and how it is managed in children and young people.


Purpose of Job:

The post holder will manage and be responsible for the engagement with the young people referred into CEW project at Kings College Hospital through:

  • Delivering high quality targeted support to young people accepted onto the Clinics for Excess Weight (CEW) project.
  • Working flexibly in the community to engage young people closer to their homes and schools ensuring the service is easily accessible.
  • Engaging with young people who disengage from specialist care.
  • Help develop deliver the CEW project service for young people with excess weight. This will involve running sessions to identify their needs / focus group work.
  • Managing the engagement of the one to one support for young people aged 1021 years through their clinics (and within the community), working closely with other clinicians involved in their care.

Specific Duties:


  • Provide needsled emotional, social and practical support to young people diagnosed with diabetes.
  • Proactively contact, engage and inspire young people to participate assessing their needs and offering a personalised approach to include face to face meetings, home visits, telephone / video support, or social media as required.
  • Motivate, empower and encourage young people to take positive action to improve their health and wellbeing, by connecting with others, attending groups, promoting selfcare, volunteering, accessing advice and information and support services. Set goals and develop plans with young people to help them take control of their health and wellbeing.
  • Work with young people in a supportive, holistic way to address practical and psychological barriers.
  • Maintain regular, supportive contact to address issues as they arise and ensure young people progress and achieve their goals.
  • Develop and maintain a directory of community resources including health, social, economic, recreational, employment and educational resources aimed at supporting families and young people to access and utilise these services.
  • To record and evaluate outcomes while continuing to develop the role.
  • To join in developing groups for families and parents with other members of the team. These may include education, recreation and parenting groups.
  • To understand when to seek advice / assistance / report back to relevant staff.
  • To take responsibility for own workload and prioritise when necessary.
  • To participate in regular supervision and appraisals.
  • To be actively involved in the delivery of formal and informal inservice training.
  • To communicate effectively with all MDT staff from consultants to nursin
