Primary Care Drug and Alcohol Service - Bridgend, United Kingdom - Cwm Taf Morgannwg University Health Board
Description
The post holder will play a key role in the continuation of the development of Primary Care Drug and Alcohol Services (PCDAS) in the Bridgend area.
This will involve working primarily alongside GP's and the wider primary care team to provide a safe and effective service to those with stable drug and/or alcohol misuse difficulties, as well as working alongside the core CDAT in the facilitation of transfer between CDAT and PCDAS services
They will demonstrate clinical leadership and facilitate the implementation and evaluation of nursing care to ensure that the highest standard of care is achieved.
They will work as part of a multi-professional team and ensure that all care is patient centred and evidenced based.
- Actively promote the development of the team utilising evidence based practice and ensure that nursing care is of the highest standard.
- Foster robust working relationships with all disciplines and substance misuse partners.
- Lead on the management of service users that are clinically stable that are ready for transition to Primary Care and supporting staff in the management of such cases.
- Support the team in the management and transfer of service users that are clinically complex and may require more intensive support from the core CDAT service.
- Ensure that a culture of effective risk management is present in the team.
- To respond and lead as required on serious untoward incidents, complaints and communications
- Deputise for Senior Nurse if required
