We are recruiting for a passionate Healthcare Assistants who wishes to join the Discharge Lounge at #TeamIOWNHS.

The Discharge Lounge strive to provide a comfortable and safe environment for patients transferring from hospital to home or onward care provision to support the timely flow and bed availability within the trust.

The Discharge Lounge accepts patients from all areas within the trust, including ED, medical and surgical wards providing excellent patient care whilst waiting for the final elements of their discharge to be finalised.


The Discharge Lounge is open from 08:00am - 20:00pm, 7 days a week, working weekends on a roster basis will be required for this role.


Assist the patient in all activities of daily living, with both direct and indirect supervision as delegated by a registered nurse.

Maintain patient comfort, dignity and privacy at all times.

Prepare patients drinks, serve meals and provide assistance as required.

Assist in the emergency resuscitation of patients, following appropriate training.

Escort patients to other areas and departments as per Trust escort protocol.

Maintain high standards of care in an environment which promotes equality and sensitivity for all individuals.


The post holder will be expected to support other clinical areas to support with relevant duties when demand for the discharge lounge is reduced in order to support patient flow.

Provide a safe and comfortable environment for patients waiting to leave hospital, providing excellent patient care.

The post holder is required to work collaboratively with the multidisciplinary team to assist in promoting patient flow within the organisation, supporting the Clinical Site team in ensuring patients are in the correct place for their care and treatment.


This role requires written documentation of all care provided to a patient and escalation to the Clinical Site Manager any delays or concerns in the patients discharge journey.

Promote a patient centred approach to care delivery.

Assist the patient in all activities of daily living, with both direct and indirect supervision (e.g. personal hygiene, eating and drinking, toileting needs, manual handling, communication needs).

Handle patients' property with care and in accordance with policy, maintaining accurate records, storing the property securely if needed i

patient medication.


Able to communicate information to colleagues, members of the multi-disciplinary team, patients and individuals involved in the patients discharges in a timely manner.

Gain a good level of understanding relating to hospital systems and processes and able to advise the patient of processes i

e:

Discharge procedures, changes in waiting times.

Prepare the environment to receive and appropriately support patients.

Order meals and liaise with the kitchen following discharges to prevent wastage.

Prioritise the daily workload in conjunction with the Clinical Site Manager to meet patients' needs promptly and effectively.


Transfer patients to and from ward areas and departments to facilitate safe patient care and promoting effective patient flow across the site.

To update Clinical Site bed management system on a real time basis to support patient flow across the site.
