GLL is looking for a Health Referral Coordinator based across our Welwyn and Hatfield Partnership.

Healthwise is our GP Referral Scheme, designed to support a wide range of people. NHS patients can be referred for many reasons from diabetes and heart disease to depression, dementia or post-operative recovery. Rehabilitation or weight loss are just the start.
